The answer is human error. It is because there are things that humans make, taking can lead to accidents or collisions. As when the person does not follow the rule of being cautious when crossing the road and railway, when the person ignores the rule and have to stop and check both ways before going through, when he or she does not see that the train in which she or he cannot see, he or she is likely to lead to an accident.
